An enemy codenamed ''Quake'', who is believed to be from another dimension, uses teleportation gates to invade Earth. The player takes on the role of an unnamed soldier who arrives at his base to find that Quake has invaded it and killed everyone. Somewhere in the base there must be a teleporter to Quake's realm. The mission is clear: lead the fight against the enemy, defeat countless hordes of monsters and take revenge.

Its main technological innovation is the use of a real 3D engine - the levels themselves as well as the enemies are polygonal.
