Hotline Miami is a violent top-down action game set in Miami, Florida in 1989. The protagonist is a hitman, nicknamed Jacket by fans, who receives messages on his answering machine from mysterious employers. Most of the game consists of various missions that always start in the killer's apartment. He gets a new task via his phone, then hops in his DeLorean to get there and get the job done. After each mission there is a short story sequence where he stops at a store.

Missions always take place in buildings with one or more floors. The jacket must go in and kill everyone inside. This can be done either silently, eliminating opponents one by one, or as a hectic sequence where everyone is attacked at once. When alarmed, usually all opponents of the same level will be drawn to Jacket. There are different ways to approach opponents. Enemies can be eliminated with melee weapons or rifles. They can also be knocked out by punching them or slamming a door in their face, but they will get up. When only stunned, Jacket can perform a brutal finishing blow or pick them up to use as a human shield. Enemies include regular guards but also dogs. Usually there is no starting weapon and they must be taken from the environment or from opponents. Some of them are location specific, like a boiling pot in a cooking environment. Other melee weapons are baseball bats, knives, crowbars and more. Firearms usually have a limited amount of ammunition. When silently eliminating enemies, line of sight and sound must be taken into account. In some missions, Jacket can cling to ledges to move outside to another part of the building. He must also take into account glass and transparent surfaces because enemies can see them and shoot through them. All weapons can also be thrown at opponents, usually stunning them, but only one weapon can be carried at any time.
