In Akuji: The Heartless, the player takes on the role of Akuji, a voodoo priest. The game's title is meant to be taken literally, as Akuji's wedding day posed a problem: his brother killed him and ripped out his heart. Akuji must now travel through the underworld to return to the living and exact his revenge.
This game is a typical third-person action game: the player jumps and fights through the stages. The player can increase their health bar by collecting voodoo dolls that are scattered throughout the levels. The camera is positioned in an over-the-shoulder perspective but can be manually adjusted. The game's levels are mostly horror-themed.
