Akai Katana is a top-down shooter with the Bullet Hell gameplay style, which means the ship has a specific area called a "hit box" that must be maneuvered through a maze of bullets and enemies. This shooter has six stages and three different game modes called Origin (original arcade mode), Slash and Climax. They each have different play styles and difficulties. In Origin and Climax, the player can switch between a defense mode and an attack mode. In defense, ships move faster, destroying the enemy at close quarters and collecting energy. In attack, the ship moves more slowly but the shots are more powerful. The collected energy then orbits the ship and increases as enemy bullets pass over it. Energy is collected when returning to defense. Energy is used to activate ghost mode for enhanced abilities. Slash mode has a different system where energy is replaced with steel orbs that can be transformed into katanas.
Players control a rebel pilot with an airplane in the style of the WWII era and go after his old masters. There are three different pilots available, each with their own ship and power. A pilot has three lives, so not all bullets are lethal, and there are also three smart bombs that clear the entire screen. Players have access to a support character for additional damage, but its use is different depending on the game mode chosen. The game offers instructional videos in order to explain the scoring system and the general concept behind the "Bullet Hell" shooters.
