CT Special Forces: Back to Hell is the sequel to CT Special Forces and the story picks up where the last game left off. After destroying the terrorist group Network, the CT special forces return to do training missions, but more and more reports of new terrorist threats continue to emerge. Could this be a new terrorist organization, or maybe Network is back for a second round? It's up to CT Special Forces to stop dealing with terrorism once again!
The game is played in 5 different game modes. The majority is made up of side-scrolling platforming action, somewhat similar to Metal Slug games, where you scavenge various weapons, grenades, and even a tank to crush the terrorist threat. Occasionally you will come across a wooden sign with a crosshair on it. This will change the game to first person view, where you have to shoot terrorists and rescue hostages. Sometimes you will find a parachute bag and you will have to play a mini-game where you have to line up two meters to successfully drop with your parachute.
There are also two types of helicopter stages. One is a top-down shoot 'em up, where you have to bomb ground targets and shoot down enemy helicopters. The other is a side-scrolling helicopter rescue game, where you have to rescue hostages and bring them to your base, while gunning down enemies across the stage.
